Shipping Instructions

Completing the Form

On the forms page, select the form that fits the request you are making. Completely fill in the general information at the top of the form. Fill in the rest of the information as completely as possible. This is necessary for accurate recommendations, if required. These points are particularly critical. Be sure to:

  • Indicate which tests are desired.
  • Indicate the intended crop(s), yield goals, and the previous crop so recommendations can be made.
  • Note any special conditions that might exist in the space for remarks so that the person making the recommendations may more fully visualize the conditions on your field.

Submitting Samples

Take to the Extension Office

You can take the samples to your local county extension office and they will forward them to the lab (postage and handling may be charged).

Ship Samples

If you are submitting samples through the mail, send packages to:

Samples may be sent directly to the lab by placing in a shipping container or wrapping in heavy paper. Information sheets should be included with the package. Label shipping container and tie securely. Discount overnight shipping is available when you purchase a shipping label online. The sender pays the postage. Do not send cash. We are not responsible for cash sent in the mail.
